If You Are An Attorney: There are two options. First, your client may contact us directly for a DMX. We will do a brief history and examination and properly document our findings. If the results suggest ligament damage, a DMX will be performed and the proper insurance billed accordingly. Second, you can also print out a Physician’s Referral form and speak with the doctor managing your client’s care.
